Privacy Collection Notice - Privacy Act 1988 (Cth)
Open Care Community Services Pty Ltd t/a Open Care Connect (ABN 22 668 873 694) collects your personal information on this form for employment commencement and HR administration, under the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
- What we collect: Name, date of birth, address, gender, emergency contacts, work entitlement status, and health or adjustment information.
- Purpose: Employment commencement processing, payroll setup, and maintaining employment records as required under the Fair Work Act 2009.
- Storage: These details stay only in this browser session until your final secure HR submission. They are then sent directly to HR for SharePoint filing and are not stored in the database.
- Disclosure: Accessible to authorised OCC management only. Bank details and your TFN / super completion confirmations are sent directly to the OCC payroll manager by encrypted email and are never stored in this system. We do not disclose your information to overseas recipients.
- If you do not provide this information: OCC may be unable to process your employment commencement or meet its employer obligations.
- Access & correction: You have the right to access and request correction of your personal information held by OCC. Contact: admin@opencareconnect.com.au
- Complaints: Contact OCC in the first instance. Unresolved complaints may be referred to the Office of the Australian Information Commissioner at oaic.gov.au or 1300 363 992.
Use ATO online services through myGov to complete the new employment tax and super details, then return here and confirm how OCC will receive the summary or paper fallback forms.
Use the OCC ABN to find the employer, leave the Employer branch ID as shown by ATO unless OCC tells you otherwise, choose the correct employment type, and complete both tax and super in the same online session.
Sign in to myGov, open the linked ATO service, then go to Employment.
Choose New employment to complete both your TFN declaration and your super choice together.
Search by the OCC ABN, keep the branch ID unchanged unless OCC specifically tells you otherwise, and choose the correct employment type for this role.
Tick both options, press Start, complete the online forms, then return here and confirm both steps are done.
If you cannot use myGov or the online ATO forms, use the official ATO paper fallback guidance instead. You do not need to upload the forms here if you complete them another way and provide them separately to OCC.
Confirm your ATO steps
Upload your identity and compliance documents, then send the complete secure HR pack to OCC for review and SharePoint filing.
Upload a passport photo page. Australian citizens may use an Australian Citizenship Certificate instead.
Use this only if you do not have a passport.
Upload both sides. One accepted 70-point identity document plus Driver Licence meets 100 points.
Visa holders only. Upload a VEVO screenshot showing work rights.
Compliance evidence groups
- Identity: passport or citizenship certificate plus driver licence; OCC records the checked date after review.
- Renewals: NDIS screening, WWCC when required, Police Check issue date, licence, car insurance, and registration.
- Completion evidence: NDIS orientation, Certificate III or IV, and infection control completion dates.
- First Aid: upload your current certificate and enter the expiry date shown on it. Your certificate must be current to commence support work.
