Authority to Act as an Advocate
This form authorises a nominated person to act as an advocate for a participant in specified circumstances. This authority does not grant power of attorney or legal guardianship, and it does not replace the participant's right to make their own decisions.
OCC will only accept advocacy in the areas selected in this form unless the authority is updated. The participant may withdraw the authority at any time by notifying OCC in writing.

This authority applies only to the situations selected below. OCC will always first seek to understand the participant's own view before acting on the advocate's instructions.

E - Consent and Signatures
By signing below, the participant confirms they freely and voluntarily authorise the nominated person to act as their advocate in the areas specified. The advocate confirms they accept this authority and understand their obligations.
