NDIS Support Plan
This Support Plan is a living document developed in collaboration with the participant and/or their representative. It translates the participant's NDIS plan goals into specific, achievable supports delivered by OCC. All support must align with the participant's NDIS plan funding, stated goals, and personal preferences. Review at least annually, after any significant life change, or when the NDIS plan is renewed.

Section C: What Is Important to This Person
This section captures the participant's own voice: what matters most to them. Use their words wherever possible. Ask open questions: "What makes a good day for you?", "What is most important in your life?", "What would you like to achieve?"
Section D: Goals and Support Strategies
Link each goal directly to the participant's NDIS plan. For each goal, record the support strategy, who will deliver it, and how progress will be measured.

Consent: The participant and/or their authorised representative confirms this Support Plan accurately reflects the agreed supports and has been developed with their genuine participation. This plan may be accessed by OCC support workers delivering services to this participant.
