In line with legislative requirements, Open Care Community Services Pty Ltd t/a Open Care Connect will be undertaking a routine quality management audit on [audit dates] with [auditing body]. [Auditing body] is an Approved Quality Auditor accredited by JAS-ANZ and appointed by the NDIS Commission to perform Certification audits against NDIS Practice Standards. Why are we writing to you?
We invite and encourage you to take part in this audit process. It is important that you have an opportunity to help identify what we are doing well and what we could do better. The auditors value your feedback in forming this picture.

As part of the audit, [auditing body] is required to undertake sampling of our service delivery. For this to happen they need your consent to:

  • Review your files: this allows us to demonstrate that we follow the procedures in our policies.
  • Interview you: this allows the auditors to find out how well the service is working for you. Please tell us if you prefer to nominate a family member or another person to provide feedback for you.

What is the audit for?
The audit is to give us information and assurance that we are complying with service and quality standards. It is not about funding.

What is included in the audit?
This Certification audit will include verification of NDIS Practice Standards. This includes service user rights and responsibilities, organisational management, service provision and work environment. Most importantly, the auditors will interview people receiving services who agree to be interviewed. The auditors will also review documents, observe how things are done and interview staff.

How does it work?
Interviews are either conducted by phone or face-to-face. It is your right to have an advocate or support person of your choice with you during the interview. Information that you give to the auditors during an interview will form a part of their audit records. You are not identifiable and these records remain confidential. They are kept securely and destroyed after 3 years.

What happens next?
If you agree to participate in the process by way of interview and file review, you do not need to respond to this letter and this will be taken as your consent.
